Through our Health 360 Partnership, Honor Community Health (HCH) has primary/preventative care service offices at our North Oakland Health Center (NOHC) and South Oakland Health Center (SOHC):

• Reproductive health services - Birth control, STI screening, basic infertility, pregnancy testing, etc.
• Chronic disease management (high blood pressure, diabetes, asthma, etc.)
• Women's health
• Pediatric services
• Dental services (limited onsite/appointment only)

Anyone is eligible for services.

Appointments

Walk in to the NOHC or SOHC during HCH clinic hours or call 248-724-7600 to schedule an appointment.

• NOHC: Monday - Friday 8:30 a.m. - 5 p.m.
• SOHC: Monday - Thursday 8:30 a.m. - 5 p.m.

Fees

There are costs associated with receiving care. HCH provides financial options, including a sliding fee schedule, and its Honor Cares Program which provides discounted medical and dental care, and enrollment services to help with financial costs.
