About

The Maternal and Child Nutrition Home Visiting Program provides home-based nutritional guidance, education, and support led by a Registered Dietitian Nutritionist (RD/RDN). 

  • Conduct individualized nutrition assessments during pregnancy, postpartum, infancy, and childhood life stages.
  • Support breastfeeding/chestfeeding/human milk feeding individuals by providing education before birth and assist, encourage, and troubleshoot during baby's first year.
  • Support caregivers in the transition from breastmilk/human milk or formula to solid foods at 6 months of age.
  • Provide formula recommendations, as needed.
  • Evaluate feeding skills and behaviors in infants and children.
  • Obtain in-home height and weight measurements to monitor growth trends.
  • Provide referrals to other maternal child health programs and food resources.

Eligibility

Must be an Oakland County resident who is considered nutritionally at risk and are one of the following:

  • Mother who is pregnant, postpartum, or breastfeeding
  • Infant
  • Child (up to age 18, must be diagnosed with a medical condition that effects nutrition)

"Nutritionally at risk" means being on the following:

  • Living with a health problem, medical condition, diet deficiency or other issue that can affect the health of a participant,
  • Living in an area with a high social vulnerability, or an area with limited access to affordable and nutritious foods, 
  • Having a household income that's below the federal poverty level,
  • Qualifying for SNAP, WIC, and/or Medicaid. Enrollment in these programs is not required.

Appointments

Appointments are scheduled by RDNs/RDs once the client is accepted into the program.
