About

  • CSHCS provides client centered, family focused, and community-based assistance to individuals with chronic (long-term) and severe medical issues.
  • Financial assistance may be available for individuals enrolled in the program to help cover the costs of:
    • Medical treatment
    • Specialist care
    • Bills, co-pays, and deductibles
    • Prescriptions
    • Medical equipment
    • Medical supplies
    • Transportation and mileage
  • Care can be coordinated and managed by working with specialized Public Health Nurses who help families:
    • Develop a plan to coordinate and manage the individual's care
    • Utilize the financial benefits of CSHCS
    • Connect with community resources

Eligibility

  • Birth to age 26
  • CSHCS covers Cystic Fibrosis, Sickle Cell Disease, and hemophilia and related coagulation disorders for adults over 26
  • Chronic medical condition(s) that meet enrollment requirements
    • CSHCS covers over 2,700 medical diagnoses
  • All income levels can apply

Appointments

Appointments are scheduled with a nurse once a client is accepted into the program.

Fees

There are no fees for clients that have active full Medicaid, MIChild or Healthy Michigan Plan. Families with commercial insurance, no insurance, or other types of medical payment program may pay a program fee to the state based on a sliding scale.
