Oakland County is inviting employees to participate in this year's health screening program. To participate, employees will need to schedule and attend their yearly well-being visit with their primary care physician and complete their online health assessment through BCBSM. Onsite screenings will not be offered.

To complete your health assessment please click the appropriate button below based on your insurance coverage through Oakland County. Employees will receive their $100 incentive when they complete both their physical and health assessment (BU's may have a different incentive).

Employees must complete their annual physical between January 1 - December 31, 2024.

Note:Acute care clinics (minute clinics & urgent care clinics), OB/GYN and specialty appointments do not qualify as a primary care visit. 

Employees must attend their primary care physician visit and complete their online health assessment by December 31, 2024 in order to receive their incentive.
