Oakland County is inviting employees to participate in this year's health screening program. To participate, employees will need to schedule and attend their yearly well-being visit with their primary care physician Click here to learn more about what are healthy numbers for your physical results.

Employees will receive their $100 incentive when they complete both their physical and health assessment (BU's may have a different incentive) between January 1 - December 31, 2024. 

*NEW* As of 4/22/2024, you will only need to attend your yearly well-being visit and take your online health assessment to get your incentive. The Primary Care Physician (PCP) form is no longer required. The Human Resources Benefit Team and Blue Cross Blue Shield of Michigan (BCBSM) will do the rest!

How to complete your Health Screening:

Step 1: Attend your yearly primary care physician visit.

Step 3: Go to Blue Cross Blue Shield website to complete online health assessment. (Click here for directions.)

Step 4: Receive your incentive.

Complete by July 26, 2024 and receive the incentive on August 30, 2024 paycheck.
Complete between July 27 - December 31, 2024 and receive the incentive on January 31, 2025 paycheck.

NOTE: Acute care clinics (minute clinics & urgent care clinics), OBGYN, and specialty appointments do not qualify as a primary care visit.

If you are on Blue Cross Blue Shield through your spouse or parent please visit the No Coverage/New Employee page for information on how you should complete your physical and online assessment. 

If you and your spouse/child are both county employees and on Blue Cross Blue Shield insurance then the primary member must create an online account for other individuals on their insurance before each of you will be able to complete the assessment. The dependent on the account must complete the PCP form and will need to visit No Coverage/New Employee page. 

CONFIDENTIALITY NOTICE:

Your personal health data will not be shared with Oakland County, or your manager, nor will it be added to any Oakland County files. BCBS will share a list of names of employees whose claims include a billing code associated with an annual physical, no other information related to billing or diagnostic codes will be included. Participation Incentive Drawings

Eligible employees must complete the screening and assessment to be eligible for a prize. Five winners will be drawn from each category.

Repeat Participant: Employees who have participated in past screenings will be entered into a drawing for a $300 incentive.

First Time Participant: Employees who participate for the first time receive a chance to win a $100 incentive.
